CHANGE OF COURSE/NAME

I wish to notify JAMB of my intention in changing my choice of institution so in essence, i would like to know the requirements for it.

Also, i wish to state that during the course of my registration (i.e UME EXAM, 2009), there was an error in my surname which when the person that made the registration went to correct it, he paid the sum of N2,500 and on checking my result, i found out that the supposed change did not take place.
Please, this is my SURNAME and i would not like it to affect my education.
